Practice relationships
Medicare enrollment and benefit-reassignment relationships between clinicians and group practices.
Directory corroboration
Clinician profiles, group locations, medical school, specialties, facility affiliations, and telehealth indicators.
Historical ACO affiliation
Performance-year provider-to-practice participation and explicit practice-to-ACO affiliations, joined only through exact CMS PAC IDs for the same year.
Facility identity by CCN
Names, addresses, telephone numbers, ownership, and certification details for hospitals, home health agencies, hospices, nursing homes, dialysis facilities, rehabilitation facilities, and long-term care hospitals.

How we keep it honest
Releases are validated and promoted atomically. Direct identifiers are preferred, exclusion findings require an exact NPI link, and ambiguous name or shared-address matches are never promoted as facts.
Appropriate use
Federal records may be incomplete, delayed, self-reported, or wrong. This is not a consumer report and must not be the sole basis for credentialing, employment, privileging, payment, or exclusion decisions.
Corrections to NPPES records must be made through the official NPPES system. Changes flow into ACOAPI with the next source release.
